1. A defender carrying the ball in-goal runs behind the referee. An attacker, the referee and ball carrier accidentally collide and all three end up on the ground. The ball spills loose and an attacker grounds the ball. What should the referee rule?

Yes, it is for the referee, and not the AR or TJ, to decide whether the ball was thrown in from the correct place (Law 6.26d)
10. Are NZDSLVs applicable to Bunnings NPC, Farah Palmer Cup, Heartland Championship, and Ranfurly Shield matches?
Yes, applicable to all levels of New Zealand domestic rugby, as specified under affected laws
